By submitting a blood sample for mail-in testing through Yoli LLC, you acknowledge and agree to the following terms and conditions: 

1. Consent for Sample Collection and Testing 

I voluntarily consent to the collection of a blood sample for laboratory testing. By submitting my sample, I authorize the testing and processing of my sample for the intended purpose. I understand that: 

• My sample will be analyzed according to a 3^rd party laboratory’s protocols. 

• Laboratory testing is not 100% error-free and may be influenced by factors such as recent illness, medications, collection technique, and shipping/handling conditions. 

• My test results are for informational purposes only and should not be used as a medical diagnosis. 

2. No Medical Advice or Patient-Provider Relationship 

I acknowledge that my test results: 

• Are not intended to diagnose, treat, or cure any medical condition. 

• Should not be considered a substitute for professional medical advice, diagnosis, or treatment. 

• Do not establish a patient-doctor or other healthcare provider relationship between me and Yoli LLC, the laboratory, or any affiliated entity. 

I am encouraged to consult a licensed healthcare professional regarding my test results before making any medical or lifestyle decisions. 

3. Sharing of Test Results with Third Parties 

By submitting my sample, I consent to the sharing of my test results with third parties as outlined below: 

• Authorized Recipients: My results may be shared with designated third parties, including healthcare providers, research institutions, program sponsors, Yoli Independent Brand Partners, or other entities authorized by me. 

• Electronic Communication: I understand that my results may be transmitted via electronic means, including email, online portals, or other digital platforms, which may carry inherent security risks. 

• De-Identified Data Use: My data may be anonymized and used for research, statistical analysis, or quality assurance purposes without further consent. 

I understand that once my test results are shared with an authorized third party, Yoli LLC is not responsible for how those results are used, stored, or further disclosed by the recipient. 

4. Data Security & Confidentiality 

Yoli implements reasonable security measures to protect my personal health information. However, I acknowledge that: 

• No system is completely secure, and unauthorized access to my data is possible. 

• I should take precautions when sharing or storing my test results. 

5. Assumption of Risk & Release of Liability 

I release Yoli LLC, the testing laboratory, and any associated parties, including their employees, affiliates, and successors, from any liability related to: 

• The accuracy, completeness, or interpretation of my test results. 

• Any decisions I make based on my results. 

• Any unauthorized access, disclosure, or misuse of my test results after they have been transmitted to me or my authorized third party. 

6. Changes to This Disclaimer 

Yoli reserves the right to update or modify this disclaimer at any time. Any changes will be effective immediately upon posting. I am responsible for reviewing this disclaimer periodically.
